UCHS Library Loyalty Card Program:
Step 1: Sign up for the program by completing the form below.
Step 2: Your card will be delivered to you.
Step 3: Read 5 books during the semester and complete a short survey about each book.
Step 4: Once all books have been read and surveys are completed, you will receive $5 dollars in Pal's bucks
*You may only complete one card each semester*
